Passport Renewal

Attached you will find the passport renewal application form and a list of the required documents:

  • Please be advised that you must send your original passport and the completed application form to the Consulate General.
    Please sign on the first page of the application form exactly in the center of the signature box and on the 3rd page on the signature line.
  • submit a complete copy of your green card (both sides of the green card)
    If you do not have a green card, then submit a letter stating “ your name and that you do not have a green card “, then sign the letter.
    The letter must be addressed to the Consulate General of the Republic of Suriname.
  • Submit your original Birth Certificate and Marriage Certificate from Suriname (with an apostille stamp). For persons born in Suriname, no Birth Certificate is required.
  • Marriage – and divorce certificates of any other foreign country must have a notarization – or apostille stamp.
    Note:  in case of a change of marital status, for example; divorce or marriage, an original legalized document must always be included.

  •  Two (2) colored photos with a white background (see attachment for instructions regarding the photos)
  • Be advised that your passport must be valid for 6 (six) months prior to renewal.
    Please apply online.

** The turnaround time is 3 – 6 months

Please be advised that in cases where there is no birth certificate or the certificate does not have sufficient information a Deed Poll can be used.

The Consulate General may conduct an interview with the applicant.

If you are applying for passport renewal for a minor child (under the age of 18 years, then parents must send to the Consulate General the following additional documents:

  • The “Letter of Consent” completed and signed by the parents of that child (see attachment)
  • Birth certificate of the child and parents
  • Copy of the biographic page of the valid passport of both parents
  • Completed application form with the requested information of the child on page 1 and signatures of both parents only on pages 2 and 3.

If the minor child cannot write or sign his/her name in the signature box on page 1, then please print “NOT ABLE TO SIGN” in the box.

Fees are payable with money orders only:

Passport Renewal $ 175.—

Damaged passport renewal fee $225.— and a signed letter explaining the reason why the document is damaged.

Lost or stolen passport renewal fee $225,– and a Police Report stating “Lost Suriname Passport and the number ”

Business passport / Zaken paspoort fee $350,–
For damaged, lost, or stolen Business passport a fee of $380,– and a Police Report stating “ Suriname Passport and the number “ and a signed letter explaining the reason for damage

Return mailing: a Prepaid envelope with a return label (addressed to yourself)
Return mailing to foreign countries: send a Prepaid envelope with a return label (addressed to yourself)

Please make the money orders payable to the Consulate General of the Republic of Suriname and print your name on the front page of the money where is stated “signature purchaser” or  “from”.

DO NOT WRITE ON THE BACK OF THE MONEY ORDER.

To request a Birth Certificate from Suriname

To request a Birth Certificate from Suriname, please send the following documents to the Consulate General of the Republic of Suriname:

  • A letter of request for a Birth Certificate addressed to the Consulate General of

  Suriname, stating your full name, the date and country of birth. Please sign

  (signature) the letter. (Please print your phone number and address in the left bottom

  corner of the letter)

  • A colored copy of the biographic page of the Suriname passport of the applicant
  • Birth Certificate fee is $70,–(money order only)
  • Return Mailing: send a prepaid envelope with a return label (addressed to yourself)

To request an Certificate of Life

To request an “Attestatie De Vita” (Certificate of Life), please send the following documents to the Consulate General of the Republic of Suriname:

  • A letter of request for an “Attestatie De Vita” (Certificate of Life), addressed to

  The Consulate General of Suriname, stating your full name, the date and country of

  birth, your current address and phone number. Please sign (signature) the letter. (Please print your phone number and address in the

  left bottom corner of the letter)

  • A colored copy of the biographic page of the valid Suriname passport of the applicant. A legalized copy of your birth certificate.

To request a Letter of Good Conduct from Suriname (Verklaring van Goedgedrag)

To request a “Letter of Good Conduct” (Verklaring van Goedgedrag) from Suriname, please send the following documents to the Consulate General of the Republic of Suriname:

  • A letter of request for a “Letter of Good Conduct” (Verklaring van Goedgedrag)

  addressed to the Consulate General of Suriname, stating your full name, the date and

  country of birth. Please sign (signature) the letter.(Please print your phone number

  and address in the left bottom corner of the letter)

  • A colored copy of the biographic page of the valid Suriname – or foreign passport of the applicant, your birth certificate, copy of your Suriname Identification card (if any) or original documents of your Suriname residence permit (verblijfsvergunning van Suriname).

.  Two (2) colored photos (white background). Twee (2) kleuren pasfotos.

  • “Letter of Good Conduct” the fee is $100,–(money order only)
  • Return Mailing: send a prepaid envelope with return labe l(addressed to yourself)

** Who may apply for a Letter of Good Conduct (Verklaring van Goedgedrag)

Please be advised that only persons registered with the Central Bureau for Civil Affairs in Suriname may apply for a Letter of Good Conduct.

To request a Marriage Certificate from Suriname

To request a Marriage Certificate from Suriname, please send the following documents to the Consulate General of the Republic of Suriname:

  • A letter of request for a Divorce Certificate addressed to the Consulate General of

  Suriname, stating your full name, the date and country of birth. Please sign

  (signature) the letter.(Please print your phone number and address in the left bottom

  corner of the letter)

  • A colored copy of the biographic page of the valid Suriname – or foreign passport of the applicant, your birth certificate, a copy of the cover and all written pages your Suriname Family Book (if any)
  • Marriage Certificate fee is $70,–(money order only)
  • Return Mailing: please send a prepaid envelope

Please be advised to register all marital status with the Central Bureau for Civil Affairs in Suriname. In case of change of marital status, for example divorce or marriage, a legalized document must always be included.
